Perimenopause and menopause

Hot Flashes, 3 A.M. Waking and Brain Fog Have a Cause

Hot flashes, night sweats, broken sleep, brain fog and stubborn weight often start years before your last period. Being told your labs are normal while you feel worse every month is exhausting and lonely. Losing your sleep, focus and confidence is not the price of turning 45.

  • Hot flashes and night sweats
  • Waking at 3 a.m.
  • Brain fog and word-finding lapses
  • Anxiety and irritability
  • Weight gain around the middle
  • Irregular or heavier periods
  • Low libido
  • Joint aches and stiffness
  • Fatigue that sleep doesn't fix

Stage 1

Perimenopause

Hormones swing and cycles change, most often starting in the 40s. Sleep, mood and focus usually shift first.

Care focus: find the drivers behind your symptoms and steady them early.

Stage 2

Menopause

Confirmed after 12 months without a period. Hot flashes and night sweats often peak around this point.

Care focus: relieve symptoms and decide whether bioidentical hormones fit while their benefit-risk balance is most favorable.

Stage 3

Postmenopause

Bone loss and heart risk rise in the years after the final period, even as some symptoms ease.

Care focus: protect bone, heart and metabolism for the decades ahead.

Root-cause care

What Dr. McCully Checks Before Hormones

Menopause symptoms overlap with thyroid, blood sugar and nutrient problems that a standard checkup leaves out. Dr. McCully rules them in or out first, so your plan treats what is actually going on.

  1. Thyroid conversion

Free T3 and reverse T3 show whether thyroid hormone is working, which a TSH result alone hides.

  2. Blood sugar and insulin

Fasting insulin, A1C and a continuous glucose monitor catch the swings behind night waking and midsection weight.

  3. Nutrient status

Ferritin, iron and vitamin D levels explain a share of the fatigue and low mood blamed on hormones.

  4. Gut health

Digestion shapes how hormones are processed and cleared, so gut problems get treated, not ignored.

  5. Hidden infections and toxin load

Lingering infections and toxin load keep the body inflamed. Mold testing joins the plan when your history points to it.

  6. Your hormones

With the drivers addressed, bioidentical hormones go to work in a body that is ready for them.

Bioidentical hormone therapy

Bioidentical Hormones, Prescribed When They Fit

Dr. McCully prescribes bioidentical hormones herself, so hormone therapy and the root-cause work happen under one roof. She steadies blood sugar, thyroid conversion, gut health and nutrient status alongside hormones, rather than treating hormones alone.

Timing matters. The Menopause Society's 2022 position statement finds that the benefits of hormone therapy outweigh the risks for most healthy women under 60 or within 10 years of menopause, which makes perimenopause and early menopause the time to get evaluated.

Real patients, real results

Women Who Felt Like Themselves Again

Women in Dr. McCully's programs describe sleeping through the night, thinking clearly and finding steady energy again, often within the first several weeks.

“I'm in perimenopause and I kept getting back normal labs. They would just send me home with no solution. Within four to six weeks, I started to feel like myself again.”

Scottie, perimenopause patient

“I'm off of the hormones. I don't have hot flashes anymore. I have more energy than I've had in years. My foggy brain has cleared up.”

“I am almost 50, and I feel like I can live another 20, 30 years feeling this way.”

Patient stories reflect individual experiences. Results vary from person to person.

Heart Risk Rises After Menopause, So Check Early

Estrogen helps protect blood vessels, so heart disease risk climbs in the years after menopause. Wellness Architecture offers the MCG® Heart Scan, an FDA-cleared, 10-minute resting scan with no needles, radiation or exercise that looks for early signs of reduced blood flow and small-vessel problems.

Questions About Starting Menopause Care

How much does perimenopause and menopause care cost?

Your discovery call is free. The 75-minute Discovery Experience costs $250, and personalized programs are priced to your plan. HSA and FSA funds are accepted for every program, and standard labs can be billed to PPO or EPO insurance.

Will I have to take hormones?

No. Dr. McCully prescribes bioidentical hormones when they are the right fit, after she looks at blood sugar, thyroid conversion, gut health and nutrient status. Some women need hormones, and some feel better once the other drivers are treated.

Should I bring my previous lab results?

Yes. Dr. McCully reviews labs from your other doctors during the Discovery Experience and shows you what they reveal about your symptoms.

Can I keep seeing my OB/GYN?

Yes. Dr. McCully works alongside your OB/GYN and primary care doctor rather than replacing them.

How often will I come in?

Programs run 5 to 16 weeks, with treatment sessions in the Menlo Park clinic once or twice a week. Women who live farther away can ask about virtual programs.

Am I too early or too late to start?

Neither. Perimenopause symptoms often begin in the early 40s, and women after menopause still gain from protecting bone, heart and metabolism. Start when symptoms affect your sleep, work or relationships.
