By that definition, it is a relatively narrow category that may or may not be treated depending on the degree of TSH elevation and the physician's judgment.

In functional and naturopathic medicine, the concept of subclinical thyroid dysfunction is broader and more clinically useful. It describes any state in which thyroid function is insufficient to meet the body's needs, even when standard markers do not flag it as abnormal. This includes:

How Blood Sugar Disrupts Everything

Blood sugar dysregulation is the most consistently underappreciated driver in the perimenopausal symptom picture, and its relationship to both thyroid function and hormone balance is direct and bidirectional.

When blood sugar is chronically unstable — whether trending high from insulin resistance or crashing from reactive hypoglycemia — it impairs T4 to T3 conversion in the liver and gut, the two primary organs responsible for converting inactive thyroid hormone into its active form. It elevates reverse T3, which occupies thyroid hormone receptors without activating them. It disrupts sex hormone metabolism by driving up binding proteins that sequester both estrogen and thyroid hormones, reducing their bioavailability. It drives systemic inflammation that worsens every hormonal and metabolic condition simultaneously.

When a perimenopausal woman is given HRT and her symptoms improve and then return, blood sugar is one of the first things to look at. Estrogen has a stabilizing effect on blood sugar and insulin sensitivity. When estrogen is replaced, blood sugar may temporarily improve, masking the underlying dysregulation. When the effect wears off or the dose is adjusted, the blood sugar problem reasserts itself, and the symptoms return.

The adrenal glands produce cortisol, the body's primary stress hormone, as well as DHEA, which is a precursor to both estrogen and testosterone. One of the most useful things I look at when a new patient comes in is her current medication list. Not to judge it, but because it tells a clinical story about what has been treated symptomatically over the years without the source being found.

A woman on an antidepressant for mood changes that began in perimenopause may have thyroid dysfunction or estrogen decline driving her depression. A woman on sleep medication for insomnia that started around the same time may have cortisol dysregulation or progesterone deficiency keeping her awake. A woman on a statin for cholesterol that rose during perimenopause may have thyroid dysfunction and insulin resistance driving her lipid picture. A woman on a GLP-1 for weight resistance may have thyroid conversion impairment and blood sugar dysregulation that are making the medication less effective than it should be.

Each of these medications addresses a real symptom. None of them addresses the system that is generating it. Omega-3 fatty acids reduce neuroinflammation, support thyroid receptor function, improve insulin sensitivity, and are essential for the production of anti-inflammatory prostaglandins that modulate pain, mood, and hormonal signaling. Low omega-3 status is almost universal in perimenopausal women and its contribution to the symptom picture is almost always underestimated.

None of these deficiencies will show up on a standard thyroid panel or a routine hormone workup. They require specific testing and specific clinical attention. And without addressing them, the hormonal and thyroid interventions that are applied will consistently underperform relative to what they should produce.
