For new patients:
Each appointment includes a detailed interview, coupled with patient history, laboratory results, dietary analysis, and physical examination. After each appointment you will be given a customized plan to facilitate a return to optimal health as well as the tools to maintain it.

Please download and print the following form for your visit. For efficiency, it is necessary to fill out the following forms before you start your visit.

- Initial 15 minute phone consultation FREE
- Initial Consultation $270 for 80 minute visit
- Return Consultations $180 for 50 minute visit
- Return Consultations $90 for 25 minute visit (typically acute visits)
- Insurance: Our office does not bill insurance. Payment is expected at time of service. Cash, check and Visa or Mastercards accepted.
- If your insurance reimberses for naturopathic visits, we will give you a form with appropriate codes that you can use for reimbersement.
